Before you start completing this Form make sure you have the following documents ready:

• An updated CV
• Copy of passport / valid photo ID – only the pages include personal information
• Copy of certificates and transcripts according to program requirements.

If you are applying for a PhD/Doctorate programme you must provide:

• An updated CV (must include job responsibilities for each job; two references – academic and professional)
• A personal statement – 300 to 500 words
• Experience certificates (If any)
